Description
Razivacaine Mesylate, is a new long-acting amide type local anesthetic, pure isoleine, with anesthesia and analgesic dual role, high concentrations of ropivacaine mesylate (molecular formula C17H26N2O · CH4O3S, English name Ropivacaine Mesylate Injection) Resulting in surgical anesthetic effects, lower concentrations can effectively block the sensory nerve transmission and the impact of motor nerve is very small. Its pharmacological properties for the lower cardiac toxicity, sensory block separation is more obvious, there is vasoconstriction, so no need to add adrenaline. Clinical mainly for surgical anesthesia, local anesthesia and acute pain control.
Pharmacological effects
With anesthesia and analgesic dual role, high concentration can produce surgical anesthesia, lower concentrations can effectively block the sensory nerve transmission and the impact of motor nerve is very small. Its pharmacological properties for the lower cardiac toxicity, sensory block separation is more obvious, there is vasoconstriction, so no need to add adrenaline.
Clinical application
1. Local anesthesia
2. Epidural block.
3. Peripheral nerve block.
4. Subretinal subcavity.
5. Local intravenous anesthesia.
6. Postoperative analgesia.
7. Obstetric anesthesia.
8. Labor analgesia.
Adverse reactions
The adverse effects of ropivacaine are similar to those of other long-acting amide-type local anesthetics. In addition to accidentally injected into the blood vessels or excessive and other unexpected events, local side effects are almost rare. To distinguish it from the physiological response caused by the blockage of the nerve itself, such as epidural anesthesia when the blood pressure and bradycardia. Overdose and careless injection into the blood vessels can cause severe systemic reactions. Allergic reactions: rare for amide topical anesthetics (the most severe allergic reaction is anaphylactic shock). Neurological complications: neurological disorders and spinal dysfunction (such as pre-spinal vascular syndrome, arachnoid mawei syndrome) and regional anesthesia, and almost no local anesthetic. Acute systemic toxicity: Ropivacaine causes acute toxicity only if the drug plasma concentration rises sharply or is excessive in the drug at high doses or accidentally injects the drug into the blood vessel. There was a case of patients with nerve ganglion due to block, inadvertently 200mg drugs into the blood vessels, the occurrence of convulsions. Most of the anesthesia-related adverse reactions are associated with the effects of nerve block and clinical conditions, and are rarely associated with drug reactions. The incidence of hypotension in patients undergoing clinical studies was 39% and the incidence of nausea was 25%. General clinical reporting of adverse reactions (> 1%) is hypotension, nausea, bradycardia, anxiety, hypoesthesia.
Contraindications
① Amide anesthesia amide anesthesia disabled.
② Severe liver disease patients with caution.
③ Hypotension and bradycardia with caution
④ Chronic renal insufficiency associated with acidosis and low plasma protein patients with caution.
⑤ Elderly or with other serious illness that is required to use regional anesthesia in patients before the implementation of anesthesia should try to improve the patient's condition, and appropriate to adjust the dose.
Precautions
The implementation of regional anesthesia must be carried out on the basis of improved staff and equipment. Drugs and equipment for monitoring and emergency recovery should be readily available. In the implementation of a large anesthesia should be given to the patient before the establishment of intravenous access. The relevant clinical staff should be properly trained and familiar with the side effects, systemic toxicity and other complications of diagnosis and treatment.
Some local anesthesia, such as head and neck injection, the incidence of serious adverse reactions is higher, and has nothing to do with all local anesthetic. Special attention should be paid to patients who are hospitalized with age or other serious illnesses. In order to reduce the potential risk of serious adverse reactions, before the implementation of anesthesia, should try to improve the patient's condition, drug dose should also be adjusted.
As ropivacaine in the liver metabolism, so severe liver disease patients should be used with caution, due to delayed drug excretion, repeated medication to reduce the dose. In general, patients with renal insufficiency who do not need to adjust the dosage with a single dose or short-term treatment. Chronic renal insufficiency patients with acidosis and hypoproteinemia, the possibility of systemic poisoning increased.
Epidural anesthesia can produce hypotension and bradycardia, such as pre-infusion expansion or the use of vascular drugs, can reduce the occurrence of this side effect, hypotension can occur once 5-10mg ephedrine intravenous treatment, if necessary Reproducible.
Effects on driving and operating machinery: Local anesthesia can affect mental condition and ataxia even if there is no obvious central nervous system toxicity, and will temporarily impair movement and flexibility, which are related to the dose.
Drugs without preservatives can only be used once, any liquid remaining in the open container must be discarded.
NOTE
Patients treated with other local anesthetics or amide-related drugs, such as the use of ropivacaine injection, should be cautious because the anesthetic effect can be accumulated.
